Changing Your NDIS Provider
A Guide from Happiness Homecare
At Happiness Homecare, we respect your choices and your right to get the support that works best for you. If you decide you want to change providers, We are here to help make the process as smooth as possible.
1. Your Choice, Your Rights
· You have the right to choose your NDIS provider.
· If you feel that another provider may better suit your needs or preferences, you can change at any time.
2. Check Your Service Agreement
· Please review your Service Agreement with your current provider.
· There may be a notice period (often 2 weeks) you need to give before the agreement ends.
· Make sure you understand any steps, like giving written notice or filling out a form.
3. Let Your Current Provider Know
· Inform your current provider (by phone, email, letter or in person) that you want to end their services.
· Be clear and polite. You do not need to say why you are leaving unless you want to.
· Ask for written confirmation about your end date and any final steps.
4. Make Sure Your Supports Continue
· Before you leave your current provider, make sure your new supports are ready to begin so there are no gaps in your care.
· Happiness Homecare can help coordinate your transition for a smooth handover.
5. Collect Important Information
· Ask your old provider for copies of your support plans, assessments, or any important documents.
· Return any equipment that belongs to them.
· Finalize any unpaid bills.
6. Start with Your New Provider
· Choose a new provider (like Happiness Homecare!) and ask about their services and how they can help you.
· Sign a new Service Agreement so your supports can start as soon as possible.
7. Inform the NDIS
· If your plan lists your old provider, let your Support Coordinator, LAC, or NDIS Planner know about the change.
· This ensures your records are up to date and your funding is used correctly.

· If you have questions or need help at any point, ask your Support Coordinator, LAC, a family member, or someone you trust.
· You can also contact the NDIS on 1800 800 110 or the NDIS Quality and Safeguards Commission if you have concerns.
